All Performance Management systems will have an email notification sent to the participants. The email example below contains just a few instructions on how to log into the system.

The employee(s) below have completed their self assessment as part of the Performance Assessment process. You have already begun to enter the Manager Assessment. Once you have finished your assessment, press the "Submit for In-Person Review" button so that the In-Person review can take place.

You can log into the system following the instructions below:

  • Log into the website by clicking on this link: https://hr-survey.com/project/
  • Your Email Address is: [Email]
  • Your Initial Password is: 123456

    You must use this initial password to access the system. You will then be prompted to reset your password on your first log in. Please choose a password which is secure and unique.
If you forget your password, click the "Forgot your password" link shown on the login screen.
